Position Summary
The Family Wellness Coordinator advances the mission of Mom’s House by supporting single parents and their children through a holistic approach to family wellness, education, and empowerment. This position designs and implements wellness programming, provides case management support, and fosters collaborative relationships among parents, educators, and community partners. The Coordinator plays a key role in strengthening family stability, promoting emotional well-being, and helping families break the cycle of poverty through education and whole-family care.
Primary Responsibilities
Parent Education & Wellness Programming
- Plan, coordinate, and facilitate parent education sessions aligned with the Mom’s House Family Wellness framework.
- Develop and maintain a network of community partners, facilitators, and guest speakers to enhance programming.
- Design and administer pre- and post-class assessments to evaluate participant growth and inform continuous improvement.
- Analyze program data to identify emerging family needs and recommend new wellness initiatives.
Client Support & Case Management
- Conduct regular one-on-one wellness check-ins to assess family progress and update individualized Family Care Plans.
- Collaborate with internal team members and external agencies to connect families with counseling, healthcare, housing, and other supportive resources.
- Model and reinforce positive communication, emotional regulation, and accountability for participants.
- Maintain accurate, confidential case notes and program data for reporting and evaluation purposes.
Client Engagement & Community Integration
- Ensure families complete required service hours and engage in the Mom’s House community through meaningful participation.
- Partner with the Education Coordinator to promote a safe, organized, and nurturing environment for children and families.
- Foster a culture of respect, belonging, and service that reflects the organization’s faith-based and family-centered values.
- Assist with donation organization, volunteer coordination, and community engagement or fundraising events that promote awareness of Mom’s House programs.

Qualifications
- Degree in social work or human services preferred.
- Experience with Microsoft Office and Google Suite.
- 3 years experience mentoring/coaching youth and young adults.
